Wellington Case Report. Primary Scoliosis

MEDICAL REPORT

 O.B.K , 10 YEARS, MALE

WCA/6043/2023

 

Diagnosis:  Primary Scoliosis

 

10year old boy referred from Camek orthopedic hospital on account of limb inequality and obvious gait abnormality which was noticed by parent a couple of months back become of increase concern to them.

However, there have not been associated with pain or difficulty in walking or any engagement. He also plays football in school. There is no associated bowel or urinary bladder obstruction. The rest of systemic review are normal.

Neurological findings are normal functions in lower limbs (Power, tone, reflex, sensations). The only positive finding symptoms is in the spine where middle thoracic spine is seen to be obviously taking a curvature towards the right side. But no associated gibbos. The left lower limb is also noted to be slightly shorter.

The above features suggest scoliosis. However, further investigation is required to achieve diagnosis.

 

Radiological Imaging:

XRAY Spine shows primary scoliosis centered on thoracic spine and then compensatory scoliosis centered on lumbar spine.

Details of this scoliotic deformity is however better find on CT or MRI.

MRI Thoracolumbar spine did not reveal any cord or cauda equine compression.

           CT whole spine confirmed details of the scoliotic anatomy

 

Plan: Conservative management as discussed with parents.

          To commence TLSO Brace as soon as possible.
