DIAGNOSIS:
DEGENERATIVE DISEASE OF LUMBAR SPINE
MANAGEMENT: Conservative management
The above named patient is a known hypertensive and diabetic with good control who was referred by his Cardiologist to this facility on account of worsening low back pains. Back pain is exercabated by standing for long and walking, and relieved by rest. There is no associated bladder dysfunction, sensory or neurogenic claudication. He walked to the clinic by himself. He has recently had a pace maker inserted for his Cardiac condition. He is currently on Tabs Amlodipine 5 mg daily, Tabs Metformin 500mg bd.
Neurologic examination is essentially normal.
